Functional Fitness/Fall Risk Assessment

Description:

The decline of functional abilities and falls are a major threat to our health and mobility. By using validated tests to assess your functional fitness and risk for falling we can determine the best path to increase balance, flexibility and strength, and thus, reduce physical decline and the risk of falling. Diabetes Self-Management Program

Description:

The Diabetes Self-Management workshop is given 2 1/2 hours once a week for six weeks, in community settings such as churches, community centers, libraries, and hospitals.

People with type 2 diabetes attend the workshop in groups of 12-16. Workshops are facilitated from a highly detailed manual by two trained Leaders (Dr Bohlken being one), one or both of whom are peer leaders with diabetes themselves.

Subjects covered include: 1) techniques to deal with symptoms of diabetes, fatigue, pain, hyper/hypoglycemia, stress, and emotional problems such as depression, anger, fear and frustration; 2) appropriate exercise for maintaining and improving strength and endurance; 3) healthy eating; 4) appropriate use of medication; and 5) working more effectively with health care providers.
Participants will make weekly action plans, share experiences, and help each other solve problems they encounter in creating and carrying out their self-management program. Call Dr. Bohlken for more information related to this workshop

Chronic Condition Self-Manangement Program

Description:

CDSMP is a six week workshop conducted for 2 1/2 hours once a week in various community settings. The program was developed by Stanford University to help people with a variety of chronic diseases (arthritis, heart disease, asthma, diabetes, and lung disease) learn the skills necessary to self-manage their disease. People with different chronic health problems attend the program together to learn and practice strategies for self-management. The workshops are very interactive allowing participants to learn and share with their peers. Workshops are facilitated from a highly detailed manual by two trained leaders.

Subjects covered include:

  1. Techniques to deal with frustration, pain and fatigue
  2. How to exercise safely
  3. Medications
  4. Communication techniques for family and medical team members
  5. Nutrition
  6. Making informed treatment decisions.
